Please accept my entry in the Grand Haven Offshore Challenge Pro-Am Sport Fishing Tournament. I agree to hold the Offshore Challenge Committee, and all sponsors, their officers, directors, agents or employees, harmless and blameless from any and all liability of any kinds for injuries, damages, loss of life, disfigurement, dismemberment, pain and suffering by the registrants and their crews during the entire time the tournament is being held. Registrants agree to be held strictly liable for any liability arising from this tournament. All captains are required to sign this form as certification that they have read and agree to abide by all rules in exclusive. No entry shall be accepted unsigned. I have read all rules pertaining to this tournament and I agree to abide by the decisions of the tournament committee.
